Replacement of Passports Information

IF THE PASSPORT WAS LOST, STOLEN OR DAMAGED THE APPLICANT IS REQUIRED TO SUBMIT THE FOLLOWING:

The completed application form
The original birth certificate with name
Proof of Barbadian citizenship
Two passport-size photographs, one of which must be certified by the person who certified section (12) of the form (example JP, doctor, lawyer.)
Barbados National Registration Card
A report from the appropriate official ( Police Department, Fire Department)
A letter from applicant explaining how , where and when the passport was lost , stolen or damaged
The damaged passport.

NB. The applicant will be interviewed in respect of the above-mentioned and other documents may be requested as a result.

The process for collection

Passports must be collected by the holder of the passport except in special circumstances.

Persons collecting passports must produce their receipt and ID card or expired passport.

If a passport is being collected by a person other than the Applicant (in special circumstances):

a letter signed by the applicant giving authorization
a letter signed and stamped by a Notary Public / Justice of the Peace.

NB: All Persons over the age of sixteen (16) must submit their application in person when applying for a Barbados passport.

Regular Passports

For Persons Over Sixteen years of age

The previous passport if not applying for the first time
The completed form
Two photos
Birth certificate
Barbados ID card
Marriage certificate – if married and female
Divorce document – if divorced and female
Citizenship document if not a citizen by birth
Deed poll if applicable
Any other documents, as requested

Application Form “B”

( Minors / Persons under 16 years of age)

The previous passport if not applying for the first time
The completed form
Two photos
Birth certificate
Barbados ID of parent
Deed poll if applicable
Any other documents, as requested

Parents or guardians must submit the application on behalf of children 16 years or under, accompanied by the minor.

Use Block Letters for all names except your signature, which should be affixed in the boxes provided and in the declaration section (11) of the form. These signatures must not extend beyond the boxes. (example of a signed form )

NB: Birth Certificates must bear a seal from the Registration Department, along with the applicant’s name.
