Additional Information for Registration

Application Form C-4 completed in duplicate and certified by a Justice of the Peace or a Notary Public
4 identical passport-sized photographs -one of which must be certified by the person certifying the C-4 form
Birth Certificate
Marriage Certificate, where applicable
Divorce Document (decree absolute), if divorced
Proof of Citizenship, where applicable
Police certificates of character from countries where applicant has resided / studied / for 6 months or more after age 16
Certificate of Recognition as a CARICOM Skilled National
Letter of Verification as a CARICOM Skilled National
Documentary proof of qualifications
